*We love to take photos. I acknowledge that during our Youth Events, photos will be taken and may be used as promotional material by Oakridge Church and The Presbyterian Church in Canada.

*Of course, precautions are taken for the safety and health of your child, but in the event of accident or sickness, OPC, its staff and volunteers are hereby released from any liability. If your child requires special medication, x-rays or treatment, the parent/guardians will be notified immediately.

*I hereby give consent for my son/daughter to attend and participate in this event. I hereby, release and hold blameless, OPC, its officers, employees, volunteers from all claims and causes of action by premises, at church sponsored activities.

*I agree to direct my child to follow the instructions of personnel of the organization in charge of these activities. I also understand that if my child fails to abide by the stated rules, they may be sent home at my expense.

*I understand that I must provide payment by May 31st to hold my child(ren)'s spot(s).
